Lily was grew up in an underground fallout shelter called a Vault and lived there until her father James mysteriously disappeared. When she followed after him, escaping the overbearing and isolationist vault overseer, she discovered the world outside was an irradiated, blasted wasteland, with civilization centered into a few small towns. While initially focusing only on tracking down her father, she gradually began to expand her focus, helping people along the way and in some cases improving or securing hubs of civilization. After questioning several people around the wastes she finally locates her father, who tells her about her mother and the water purification project they'd worked on together, in the ruins of the Jefferson Memorial.

They'd not been back at work long when the purifier was overtaken by the remains of the United States government, now calling themselves the Enclave. James was killed in the takeover and Lily guided the remaining scientists to the stronghold of a group called the Brotherhood of Steel, an organization dedicated to preserving the technology of the old world and who had provided assistance to James in the past.

From there, her main two focuses were locating a piece of equipment needed to finish work on the purifier and the defeat of the Enclave. The Brotherhood pointed her in the direction of an old Vault to pick it up, but she was intercepted by the Enclave's Colonel Autumn knocking her out with a flash grenade and sending the equipment off with his soldiers to install themselves. Back at the Enclave's headquarters she's interrogated briefly, then released with an order from the Enclave's robotic President Eden: sabotage the purifier. Luckily Eden makes absolutely no effort to ensure that she actually does this, especially after she manages to talk it into blowing itself up, and with the help of a Brotherhood-built giant robot named Liberty Prime they take back the purifier, do away with Colonel Autumn, and provide free, clean water to the whole of the Capital Wasteland.

Lily had many more small adventures before being approached by an operative, and in her two years since unplugging has made a niche for herself as an enterprising thief and doctor, claiming several abandoned rooms down in the very bottom of Zion's habitable district. Her clinic, named the Warren Clinic, treats those who for one reason or another can't get time with a "respectable" doctor and runs on donations and her own 'borrowed' spoils.
